The Trilogy HAL-RAR System comes in three unique parts. First, you have the Trilogy applicator which requires 2 AA batteries to be applied before the procedure. Next, this will provide 3 bright LED lights, so you have complete visualization of the anatomy. The applicator will then pair with a bluetooth speaker, allowing you to control the volume of the doppler probe while you are locating the vessel. Last, you will attach a single doppler probe to the front of the applicator allowing precise location of the hemorrhoid and artery.
The HAL method is suitable for treating low to medium grade hemorrhoids and is extremely effective in addressing the symptoms of hemorrhoidal disease. The ligations serve to reduce the arterial blood supply, causing the hemorrhoidal cushions to shrink back to normal size. This method can be carried out with ease using the TRILOGY Probe.
Attach the Doppler probe to the Trilogy Applicator by at the base of the system, you will hear an audible click, once the two pieces are attached. Once the radio is Bluetooth paired, insert the probe into the rectum and locate the artery using the doppler. The loudest sounds indicate the center of the artery.
Once the first artery has been found, it is ligated using an A.M.I. Suture with a 5/8 needle. After successful suture placement the system is turned to locate any additional arteries. The ligation window points towards the prolapse position requiring treatment. The initial stitch is made as far proximal as possible. This is done to reveal more mucosa distally when the HAL-RAR System is rotated again.
The Final step of the Trilogy HAL-RAR System is to lift the hemorrhoid. A running suture is started, and then continued with treatment. gradual turning of the unit, leaving 7 to 10 mm between each stitch. After the last stitch, which ends at the proximal portion of the Linea Dentata. Next, the needle is cut off and the suture material knotted up near the initial stitch. This causes the prolapsing tissue to be pulled up towards the initial stitch, where it is then secured in place with a sliding knot.
